Fee Schedule
Some services are not covered by the Alberta Health Care Insurance Plan (AHCIP). These uninsured services require payment at the time of your appointment. Below is our current fee schedule. Payment is required at the time of service. We accept debit, credit cards and exact cash (We have no change or cash on site) Receipts will be provided for all services.
Payment Plans: If you are experiencing financial hardship, please ask your physician or a staff member about setting up a payment plan.
Please note: Fees are subject to change without notice. Variable costs may apply depending on the complexity or length of documentation.
- Sick Notes $35
For Work, School or Daycare
- Missed Regular Appointment $50
Charge for missed or late-cancelled regular office visits.
- Missed Physical Exam/Procedure $100
Charge for missed or late-cancelled annual/complete physical exams or procedure including IUD's
- Chart Transfer $50+
Administrative cost for transferring a complete chart to another clinic or physician. The price may change depending if you want the chart printed.
- Medical Legal Reports $100+
Detailed physician reports requested for legal or court purposes (price may vary). Your Law office may cover the fee.
- Drivers Medicals $150
Medical examination required for driver’s license renewal or commercial license applications.
- Drivers Medical (Age 75+) $85
Medical examination required for driver’s license renewal or commercial license applications.
- Pre-Employment Medical with Exam $75+
Full exam and documentation required for employment or occupational clearance.
- Aviation Medical $75+
Transport Canada-required exam for pilots and aviation staff.
- Certificate of Fitness (School / Camp) $75+
Exam and certificate confirming a patient’s fitness to attend camp, sports, or school activities.
Forms and Reports
Please note: Due to the drastic increase of forms we have been receiving from our patients, there may be a wait for forms to be completed. Please contact the clinic coordinator if you have any questions.
- Form Completion (general) $50+
Flat fee for forms not listed elsewhere.
- Long-Term Disability Form $100+
Comprehensive form completion required for long-term disability insurance or employment-related claims.
- Disability Tax Credit Certificate $150
Government form required to support Disability Tax Credit applications. Please note we have we require the payment before completion and submission.To make the application process easier, please apply directly through your CRA (Canada Revenue Agency) online account. Once you have applied, the CRA will provide you with a code to share with your physician.If you do not have access to your CRA account, our clinic already has the required DTC form available in our EMR system. Please do not bring in a paper copy.For more information or assistance, please contact our reception team.
- Disabled Parking Placard $50
Form completion for disabled parking placard applications.
- Attending Physician Statement (APS) $100
Detailed form for insurance or disability claims requiring a physician’s assessment.
- Short-Term Disability Form $50
Form completion required for temporary disability claims (e.g., illness, injury, or recovery periods).
- AISH Medical Form $75+
Completion of medical forms required to support AISH applications. $75 and upwards depending on complexity
- CPP Disability Medical Form $50+
Canada Revenue Agency covers $85 of the standard CPP Disability Medical Report form.If your case is more complex and requires additional time or documentation, an additional fee will be charged to you. Please speak with the clinic coordinator if you have questions about potential costs.
- Medication Special Authorization Forms $50
Forms required by insurance providers such as Alberta Blue Cross, Manulife, Canada Life, and Green Shield for medication coverage approval.
Out-of-Country & Uninsured Visits
Please note: Out-of-country patients and uninsured citizens are responsible for payment at the time of service. These visits are not covered under the Alberta Health Care Insurance Plan (AHCIP). Rates may vary depending on the length and complexity of the visit.
- Out-of-Country Visit – First Visit $150
Initial medical assessment for patients visiting from outside Canada.
- Out-of-Country Visit – CPX $150
Complete physical exam for patients without Alberta coverage.
- Out-of-Country Visit – Follow Up / Non-CPX $75
Follow-up visit for non-insured out-of-country patients (not including full physicals).
- Uninsured Citizen (Out of Province) $60*
If you have valid provincial health coverage in your home province (excluding Quebec), we can bill your visit directly to your provincial health plan. You will need to provide the address attached to your provincial health care card. If your coverage is invalid or lapsed in your home province, a $60+ fee will be charged depending on the type of visit.
- Resident from Quebec $60
Visit fee for patients visiting or recently relocating from Quebec (not billable to Quebec health).
